    FracFocus.org was set up in April 2011 by oil and gas companies as a place where they would voluntarily disclose information about all their U.S. hydraulic fracturing jobs. But the companies failed to report more than two out of every five fracked wells in eight U.S. states through the end of last year, according to Bloomberg data. Worst disclosure rates: CXO, SD, QEP, APA.
