One issue: cable operators have been obsessed with controlling set-top interfaces. Moreover, unlike music studios, they haven't suffered enough from online distribution to upend their business models. Sources claim Apple's tech could eventually go into in a TV set, and prototype sets have been developed - past rumors have suggested pay-TV providers are testing them. But squabbles over control and content deals could get in the way here as well.

The writing is on the wall, the question is when. Hopefully the companies that own the TV stations start realizing this and start offering ala-carte choices to buy stations.
Comcast charged me $18 a month for HBO. It was a no brainer to cancel that and purchase Netflix for $7.99 a month.
On a side note, I also feel it's a complete waste of time having to painstakingly scroll through the channel guide on my set-top box. To add insult to injury, TWC puts ads in the TV guide, and forces you to scroll through them to get to the next page. I am canceling my TV subscription next month, and will only be paying for internet access from here on out.
I "cut the cable" a week ago. Best feeling in the world. I got a boxee box with the TV dongle and Netflix. Its not the best device, but the picture quality blows away my comcast (my comcast wasn't HD).
I get all the normal UHF and VHF stations (along with TONS of other ones, I have like 4 NBC stations as compared to 1 with comcast), Netflix for movies, and then I have my home PC setup as a media server. So I can stream all my movies, no more fumbling with DVD's.
