Pfizer recalls two products

Pfizer (PFE) voluntarily recalls two lots of Effexor XR (venlafaxine HCL) and one lot of Greenstone LLC-branded Venlafaxine HCL in response to a pharmacist report that one bottle of Effexor XR contained one capsule of Tikosyn (dofetilide) in addition to the Effexor capsules. The recall is being done as a precaution.

The use of Tikosyn by an Effexor patient could potentially be fatal.

The company has notified the FDA.
