Hospital operator HCA files for an initial public offering to raise up to $4.6B, the largest IPO in two years. The chain - taken private four years ago by KKR (KFN) and Bain Capital in a record LBO - hopes to repay a significant amount of debt and raise a net $2.5B of the total by selling shares.
