Chevron's (CVX -0.5%) Richmond refinery didn’t replace an eight-inch pipe during an inspection shutdown late last year that failed and led to last weekend's fire, the U.S. Chemical Safety Board says. The eight-inch pipe is original to the unit and probably was constructed in the 1970s.

    Come on SA. You make it read like Chevron did something wrong.
    According to the article, there was no reason to indicate at the time the pipe needed to be replaced. There are thousands of pipes in a refinery and all of them don't get replaced at every inspection or you'd be paying $10 for gas.
