    Humana (HUM) gets an upgrade from JPMorgan to Overweight from Neutral (price target $91 from $88) following last week's beat and raise. "What's a reasonable [minimum] return on invested capital metric that we should think about for the Medicare Advantage business?" analyst Justin Lake asked on the call. 13% "is the number that it is today [and] we expect to improve on it, but not in a predictive sense," management responded.     Goldman Sachs opts to remain selective on a few names in the managed care sector following yesterday's announcement by the CMS that Medicaid Advantage rates were going higher. It's reiterating a CL-Buy on UnitedHealth (UNH +7.6%), calling its valuation under 10X FY14 EPS attractive, as well a Buy on Health Net (HNT +3.6%) and WellCare (WCG +5.2%). It maintains Sell ratings on Humana (HUM +8.3%) and Universal American (UAM +9.9%) however, noting the majority of their earnings are tied to Medicaid Advantage with about half of those members in non-HMO plans."

    Humana (HUM) says that the despite the proposed increase in Medicare Advantage rates in 2014, certain adjustments "may still present meaningful challenges in certain geographies." The company expects reductions under the Affordable Care Act and other factors to "nearly offset" the increase in the rates, while other adjustments are likely to hurt bid premiums in some markets. Despite Humana's caution, shares are +9.3%. (PR)

    The Centers for Medicare and Medicaid Services, after originally calling for reducing Medicare Advantage payments in February, seem to have reversed course, now projecting that combined growth in Medicare Advantage rates and fee-for-service rates will be 3.3%. Though it's unclear precisely what that may mean for health insurers, the markets are taking it as a positive: HUM +9%, AET +2.3%, WCG +4.1%, UNH +4% AH.

    Humana (HUM -2.9%) trades lower today after Susquehanna cut the shares to Neutral and lowered its target price to $74 from $85. The firm says it's opting to stay away from the potential volatility, believing the shares could swing wildly between the high-$40s to the mid-$80s depending on how the final 2014 Medicare Advantage rates - due to be released April 1 - play out.

    Humana (HUM -2.3%) says its CFO James Bloem will retire at the end of the year, prompting the health insurer to launch an internal and external search for his successor. Jefferies says the move comes at a challenging time for the company, and Bloem's successor is stepping into a difficult role, which could raise some concerns. It's only been a year since Bruce Broussard took the reins as CEO, and with worries over lower Medicare Advantage rates next year, the announcement "adds more uncertainty to management's ability to navigate the rate headwinds," Jefferies says.

    Humana (HUM -9%) says that preliminary 2014 Medicare Advantage benchmark payment rates, which were announced on Friday, will fall by mid-single digits. The company had expected the Medicare rate to be flat to slightly down, and it's now working out how it's going to pull off the trick of growing its Medicare membership and its earnings in 2014. (PR)

    Humana (HUM -2.2%) trades lower today on reports of some end-of-year related insider selling. Separately, the company announced that it's established a partnership with DuPage Medical Group to launch the BreakThrough Care Center. HUM touts the center as a comprehensive, holistic outpatient clinic designed to improve medical outcomes while lowering health care costs and improving patients' ability to manage their health conditions.

    Humana (HUM -8.1%) slides on a couple of  post-election downgrades. Goldman Sachs cut the managed care provider to Sell, saying it holds a precariously high exposure to Medicare Advantage, which heightens the stocks regulatory risk and uncertainty following the President's victory. The firm also trimmed its price target to $62 a share from $77. Separately, BofA Merrill Lynch also downgraded the shares to Neutral.

    Insurers are taking an even more cautious stance on Questcor's (QCOR -4.1%) Acthar, according to a Citron Research report. The short seller is now claiming that Humana (HUM -1.5%) has issued a new Commercial Preauthorization document containing a new, specifically asterisked preauthorization for reimbursement for Acthar, which it contends is part of a pattern of insurers taking a more restrictive stance on authorization policy with regard to the drug's reimbursement.

    Shares of Humana (HUM -13%) get slammed this morning, a day after the managed-care provider missed the mark on Q2 earnings and cut its profit outlook for the year. The dismal report is taking down more than one insurer that otherwise might have had an up day: Aetna (AET -1.9%), WellPoint (WLP -3.5%), UnitedHealth (UNH -3%).
